The person signing this form (the student, participant, or parent/guardian if under 18) voluntarily agrees to participate in one or more activities, events, programs, or productions operated or sponsored by Full Sail University (the “University”) or its representatives, including but not limited to media projects involving video recording, photography, or interviews. The participant understands that these activities may involve physical or creative tasks and may carry certain risks, including the potential for injury, property damage, or other unforeseen consequences.

In consideration of being granted permission to participate, the participant releases and holds harmless the University, its board, officers, employees, agents, volunteers, students, and affiliates (“Released Parties”) from any and all claims, costs, losses, damages, or liabilities of any kind—known or unknown—that may arise out of or relate to participation in any of these activities, including those caused in whole or in part by the negligence of any Released Party.

The participant further agrees to indemnify and defend the Released Parties against any legal actions, attorney fees, or other expenses resulting from their participation. This release also authorizes the University and its affiliates to record, photograph, and use the participant’s image, voice, and likeness in any format, for educational, promotional, or media purposes, without compensation or restriction, and without any obligation to notify the participant in advance.

By signing this form, the participant acknowledges full understanding of this release, accepts all associated risks, and confirms that they are legally authorized to grant such rights and permissions.
